Problem Definition (Definición del Problema)

Problematic: The training equipment and expenses for the minimally invasive surgeries are high cost. The trainings are based on several activities that permit a simulation of a surgery, just like the instrument manipulation. But, there is a gap between the training and the real practice. The lack of training accessing into the abdominal cavity is one of the most important indicators when a surgery is realized. Around 80% of the accidents on lap surgery appear to happen during the very first incision that drives to the entrance of the abdominal cavity.

Problemática: El equipo y los gastos de entrenamiento para las cirugías mínimamente invasivas son de costos elevados. Los entrenamientos constituyen una serie de actividades las cuales permiten a simulación de la practica quirúrgica, como son el manejo de la instrumentación. Sin embargo, existen brechas entre el entrenamiento y la práctica real. La falta de entrenamiento de acceso a la cavidad abdominal se presenta como un indicador importante a la hora de las cirugías. Alrededor del 80% de los accidentes en cirugía laparoscópica se presentan en la primera incisión para entrar a la cavidad abdominal.

 

Opportunity: Now days the 10% of 15 million surgeries in the USA are done by MIS; in Europe is expected that in 5 years time the 25% of all the operations are going to be done by MIS; in Latin America and Colombia, the percentage grows to 1% of all operations. The savings for the hospital is referenced in maintaining the patient the less time after the operation, although the price for the MIS equipments is high. This tells us that the training index is also going to grow, and there is going to be a need of equipments and training platforms.

Oportunidad: Hoy en día el 10% de 15 millones de cirugías en EEUU se hacen por MIS; en Europa se espera que en 5 años el 25% de todas las operaciones se hagan por medio de MIS; en Latinoamérica y Colombia el porcentaje crece al 1% de todas las operaciones. El ahorro para hospitales se ve referenciado en mantener la menor cantidad de tiempo que se mantiene a un paciente en post-operatorio, aunque el precio de equipos para MIS sea alto. Esto indica que el índice de entrenamiento también va a crecer, y se necesitan equipos y plataformas de entrenamiento.

 

Concept: This mapping allows us to locate the different possibilities of activity, depending on the expertise scale (y axis) and fidelity (x axis). The expertise is measure in terms of the psychomotor skills; and the fidelity, in how close is to the surgery reality.

Concepto: Mapeo que nos permite ubicar las diferentes posibilidades de actividad, dependiendo de cual sea su grado de experticia (eje y) y fidelidad (eje x). La experticia medida en términos de las habilidades psicomotrices; y la fidelidad en cuanto  la cercanía con la realidad que se vive en una cirugía.

Surgical Reality: It is not just matter of designing object for the learning and training, but to spread experiences that contain the most quantity of factors included in the surgical procedures.

Realidad quirúrgica: No es solo cuestión de diseñar objetos para el aprendizaje y entrenamiento, sino desplegar experiencias en las que se puedan contener la mayor cantidad de factores incluidos en procedimientos quirúrgicos.

Class A Butchery: The high grade of fidelity consider to contain the most quantity of factors listed, such as: texture, thickness, surface and cavity densities; even sounds, patient position, tools and instrument location; even more vital signs and the patient health

Carnicería de alta gamma: El alto grado de fidelidad considera tener en cuenta la mayor cantidad de factores mencionados como: texturas, grosores, densidades de las superficies y cavidades; hasta sonidos, posición del paciente, ubicación de las herramientas; hasta los signos vitales y salud del paciente.

In-vitro Juggling: To have concrete activities with learning aiming, having intangible elements that allow different types of exploration, handling the tissue simulation or the instrument manipulation or an applicated technology.

Low budget Safari: It is a very close approach to the actual trainers, in which the central activity consist on motor skills. Therefore, additional factors are taken here, such as the behavior of de abdominal cavity. Searching for experimental aiming throughout the limited resources, activities exercise with recursive or/and daily elements of low fidelity.

Safari de bajo presupuesto: Este es un acercamiento muy parecido a los actuales trainers, en los que la actividad principal se centra en las habilidades motrices.Sin embargo, se tienen en cuenta factores adicionales, tales como el comportamiento de la pared abdominal. Buscando fines experimentales a través de recursos limitados, ejercicio de actividades con elementos recursivos y/o cotidianos de menor fidelidad.
